NOTICE: Any scholarship winner who fails to provide this NOTICE OF INTENT to use a ThinkQuest scholarship on or before September 30, 2007, or who fails to provide the additional required information on or before October 31, 2008, will ordinarily be deemed to have declined his or her scholarship. So if you are a scholarship winner, please complete the form below now to ensure compliance:

	I plan to use all or part of the scholarship as indicated above, and I agree to:
Notify Advanced Network & Services, Inc. promptly in the event of a change of address, telephone number, or preferred email address occurring on or before December 31, 2008, and Provide Advanced Network & Services, Inc., on or before October 31, 2008, with the final scholarship information concerning tuition or other education-related expenses sufficient for Advanced Network & Services, Inc. to determine whether the tuition or other expenses are eligible to be paid with scholarship funds and distribute the approved amounts within 60 days. I understand that failure to provide this information by the specified deadline will result in a forfeit of my scholarship. Nominate the following ThinkQuest organization (regional, national or international) to receive the scholarship monies, for any part of the scholarship that is not used prior to December 31, 2008, to be used for future ThinkQuest Awards for other students.
